Home / Who is Eligible to join the SDC Program?

It's a good idea to check with your service provider to verify ALL of the following. Or give us a call and we will answer your questions and assist you.

  1. You must be willing to join the research study (which gives you a 50/50 chance of entering the SDC program) and be willing to take part in 3 research interviews over the phone in English, and allow DSHS to release information about your services and status to the researchers at UIC.
  2. You must be a currently-enrolled Service Package 3 client in the NorthSTAR system.
  3. You must be age 18 years or older.
  4. You must be handling your own finances (i.e., not have a representative payee or legal guardian).
  5. If you are currently receiving SSDI, you cannot be using Medicare as your only health care coverage, or be planning to apply for Medicare as your only coverage. However, if you receive SSDI and are covered by both Medicaid and Medicare, or by Medicaid alone, you are eligible.
  6. You cannot be homeless, living in a shelter or other temporary housing, or residing in most mental health agency’s housing; however, residents of Metrocare housing ARE eligible.
  7. You cannot be abusing or dependent on substances unless you are enrolled in and actively participating in drug or alcohol treatment.
  8. You cannot have been involved in the criminal justice system for certain types of crimes, such as assault or bodily injury to others.
  9. You must be planning on remaining in the NorthSTAR area for the next 2 years.
